1. Fill up the application form by clicking the "Request School Records" button below. Keep the application no. that is generated after submission of your document since you will need it to track the status of your application.
2. Once the application form has been filled up and submitted,you can track the status f your application by clicking the "Check School Records Request button). Provide the required details and click the "Submit" button.
3. Check the status of your application. If the status is " Approved by the Accounting Staff", you may proceed with the payment of the corresponding fees for the requested document. You will also receive an email when it is okay for you to proceed with the payment as well as the amount to be paid.
4. After payment of the fees, you may check the status when the documents are ready for pick up or when the documents are mailed via the designated courier with the specific tracking number.
If the application is disapproved by the Accounting Office due to unsettled previous balance, make/submit another application after payment has been made for the unsettled account or balance.
